Email of record Must match the email registered for your DentBooks account. Requests from other addresses are rejected for identity verification.

Practice name Optional, but helps us locate your records faster.

3 · Anything we should know?

Details (optional)

Important — federal tax law (IRS, 26 CFR § 1.6001-1) requires DentBooks to retain transaction records for 7 years from the transaction date. Deletion of an account stops collection and removes your authentication and bank-connection records, but transaction history may be preserved for the IRS retention period.
